Brief Job Overview

 
An excellent opportunity has arisen for a compassionate and proactive Occupational Therapist to join the Reablement Team at Wiltshire Council in Bourne Hill.  
This hybrid role (50% face-to-face) focuses on supporting individuals after hospital discharge to rebuild their confidence, restore independence, and reduce long-term care needs.  

As part of a dynamic, person-centred team, you will conduct strengths-based assessments, develop tailored reablement plans, and act as an expert advisor on equipment, adaptations, and safe practice. Your work will empower individuals to regain independence within their homes and communities, making a direct and meaningful impact on their quality of life.  
 

Key Responsibilities

 

  • Undertake holistic, strengths-based assessments to identify individual needs and goals following hospital discharge or a period of ill health.
  • Design and implement personalised reablement plans to support individuals in regaining confidence and independence at home.
  • Provide professional guidance and clinical expertise to reablement team members, including support workers and therapists.
  • Act as an expert advisor on appropriate use of equipment, assistive technology, and home adaptations.
  • Promote independence by reducing reliance on long-term care and encouraging recovery through rehabilitation strategies.
  • Conduct home visits and community-based assessments to ensure safe environments and continuity of care.
  • Collaborate effectively with health and social care professionals, including nurses, social workers, and physiotherapists, to coordinate reablement goals.
  • Ensure accurate, detailed, and compliant case recording in line with council and HCPC standards.
  • Contribute to service development, quality improvement, and staff training initiatives.

 

About You

 
You are an empathetic, self-motivated, and skilled Occupational Therapist with a strong commitment to promoting independence and improving outcomes for adults in the community. You take pride in supporting people to regain control of their lives through rehabilitation and strength-based practice.  

You are confident in managing your own caseload and enjoy the flexibility of hybrid working while maintaining strong communication and teamwork with colleagues.  
 

Requirements

 

  • BSc (Hons) in Occupational Therapy (or equivalent).
  • HCPC Registration as a qualified Occupational Therapist.
  • Minimum of two years post-qualification experience (ideally within reablement, community, or adult social care settings).
  • Strong knowledge of rehabilitation, equipment provision, and home adaptations.
  • Excellent communication, assessment, and report-writing skills.
  • Ability to work independently, manage caseloads effectively, and meet deadlines.
  • Commitment to strength-based, person-centred practice.
  • Full UK driving licence and access to a vehicle (essential).
  • Ability to work flexibly across office, community, and remote settings.

 

Contract & Working Pattern

 

  • Hours: 37 hours per week
  • Working Times: 09:00 – 17:00, Monday to Friday
  • Hybrid Working: 50% face-to-face, 50% remote
  • Contract: Ongoing locum
  • Salary: £36.54 per hour
  • Location: Bourne Hill, Wiltshire

The Wiltshire Council Reablement Team provides short-term, intensive support to help individuals regain confidence and independence following illness, injury, or hospital discharge. The service adopts a strengths-based, person-centred approach, empowering individuals to maximise their potential and live fulfilling lives.  

You will receive ongoing support from the Principal Occupational Therapist and have access to professional development opportunities, including leadership and coaching programmes. This role offers the chance to make a genuine difference while working within a supportive and progressive team environment.
