Brief Job Overview

 
An excellent opportunity has arisen for an experienced Band 6 Occupational Therapist to join the Intermediate Care Service at Sheffield Teaching Hospitals NHS Foundation Trust. This ongoing locum role offers a dynamic and supportive environment where you will play a vital role in promoting recovery, independence, and safe discharge for patients transitioning between hospital and home.  

You’ll work collaboratively within a multidisciplinary team, providing comprehensive assessments, interventions, and tailored rehabilitation plans to help individuals regain confidence and independence in their daily lives.  
 

Key Responsibilities

 

  • Deliver specialist Occupational Therapy assessments within Intermediate Care settings to identify individual rehabilitation needs.
  • Develop and implement goal-orientated treatment and care plans in partnership with patients, carers, and colleagues.
  • Promote safe and timely discharge planning through effective communication and collaboration with health and social care teams.
  • Recommend and arrange suitable equipment, aids, and adaptations to maximise independence and safety at home.
  • Provide rehabilitation and reablement support to promote functional recovery and prevent hospital readmission.
  • Maintain high standards of clinical documentation and ensure compliance with professional and Trust guidelines.
  • Contribute to service development and continuous improvement initiatives within the Intermediate Care team.
  • Support students and junior staff through supervision and mentoring as required.

 

About You

 
You are a proactive and confident Occupational Therapist with proven experience in Intermediate Care, rehabilitation, or acute discharge services. You thrive in a fast-paced environment and are committed to delivering patient-centred, evidence-based care.  

Your communication, teamwork, and problem-solving skills will allow you to make a tangible difference to patients’ recovery and independence.  
 

Requirements

 

  • BSc (Hons) or Diploma in Occupational Therapy.
  • HCPC registration as an Occupational Therapist.
  • Post-qualification experience in Intermediate Care, Acute, or Community Rehabilitation settings.
  • Strong clinical reasoning and assessment skills.
  • Experience with rehabilitation, reablement, and discharge planning.
  • Excellent communication and interpersonal skills for multidisciplinary collaboration.
  • Good IT and documentation skills in line with NHS standards.
  • Ability to manage a varied caseload with minimal supervision.

 

Contract & Working Pattern

 

  • Working Hours: 09:00 – 17:00
  • Days: Monday to Friday (shift-based)
  • Contract: Ongoing locum position
  • Salary: £Negotiable (dependent on experience)
  • Location: Sheffield Teaching Hospitals NHS Foundation Trust, South Yorkshire

The Intermediate Care Service within Sheffield Teaching Hospitals NHS Foundation Trust provides short-term rehabilitation and reablement to support individuals in regaining independence after illness or injury. The service bridges hospital and home care, helping patients recover and return safely to community living.
