Undertake holistic assessments of adults with physical disabilities to identify needs and promote independence.
Recommend and arrange appropriate equipment and home adaptations in line with professional guidelines.
Develop and implement person-centred intervention plans, reviewing outcomes regularly.
Work collaboratively with service users, families, and multidisciplinary teams to deliver coordinated care.
Maintain accurate records and documentation in line with HCPC standards and council policies.
Provide professional advice and support to colleagues and partner agencies regarding occupational therapy approaches and solutions.
