Provide occupational therapy assessments and interventions to adult patients within the Homefirst community service, supporting safe discharge and recovery at home
Work within an acute and community setting to promote functional independence and reduce hospital admissions
Assess for and recommend appropriate equipment and minor adaptations to support daily living and mobility needs in the home environment
Collaborate with multidisciplinary teams, including housing and social care professionals, to deliver holistic and person-centred care
Maintain accurate and timely clinical records in line with NHS and professional standards
Work shift hours from 08:00 to 16:00, with flexibility required to meet service demands
