Complete assessments and interventions for hospital discharges within a fast-paced front-door team, ensuring timely and safe transitions from hospital to home
Manage a small caseload while providing leadership and support to Occupational Therapists (OTs) and Occupational Therapy Assistants (OTAs)
Work closely with the Discharge to Assess (D2A) team or similar services to facilitate efficient care planning and reduce hospital readmissions
Conduct daily community visits and maintain a strong on-site presence (4–5 days per week) to support team operations and service delivery
Collaborate with multidisciplinary teams and contribute to effective discharge planning processes in line with NHS standards
Prior NHS experience and hospital discharge knowledge are essential for delivering high-quality, responsive care in an acute adult setting
