Job details:  Community Neuro Service. Occupational Therapist required to start asap for 3 months initially. This post is part of the Occupational Therapy team within the Extended Primary Care service in the Community Health directorate:

• Facilitating the timely discharge of patients from Fothergill inpatient stepdown ward to their own homes
• Liaising with ward staff (doctors, nurses, physio’s, SW, HCA’s) to ensure that patients identified for discharge are provided with a timely and comprehensive OT assessment
• Liaising with senior OT in the EPCT to discuss patients’ discharge needs, as required
• Understanding of community services in Newham that can support discharge planning
• Understanding of equipment and minor adaptation needs of patients in the community.

• To provide a specialist client centred occupational therapy service to patients in the community and on Fothergill Ward
• To work as an autonomous practitioner in managing a defined caseload of complex cases independently, evidencing problem solving and clinical reasoning skills in line with evidence based/ client centred principles
• To apply specialist occupational therapy knowledge and skills to the assessment of the needs of a given client group. From the assessment, to formulate a diagnosis and treatment plan designed to reduce the impact of disability and ill heath
• To provide supervision, teaching and appraisal for junior occupational therapists, assistants, students and other members of the multidisciplinary team
• To contribute to the planning, development and evaluation of the Occupational Therapy service
