As an Occupational Therapist with Croydon Health Services NHS Trust, you will provide specialist assessment and intervention to support patients in achieving independence, safety and improved quality of life. Working collaboratively with patients, families, carers and multidisciplinary teams, you will deliver evidence-based rehabilitation programmes across acute, community and outpatient settings.
Undertake detailed assessments of patients’ functional abilities and rehabilitation needs.
Plan, implement and review tailored treatment programmes in line with clinical priorities and best practice.
Work closely with patients, carers and healthcare professionals to coordinate care and discharge planning.
Maintain accurate and timely clinical documentation in accordance with NHS and professional standards.
Provide education, advice and training to patients, families and colleagues on adaptive strategies and equipment use.
Contribute to service development and quality improvement within the occupational therapy service.
