As an Occupational Therapist with London Borough of Hackney Council, you will provide specialist assessment and intervention for adults to promote independence, safety and wellbeing. Working collaboratively with service users, families, carers and multidisciplinary teams, you will develop and deliver tailored programmes that support daily living and enhance quality of life.
Conduct comprehensive assessments of adults’ functional abilities and daily living needs.
Plan, implement and review personalised intervention programmes to promote independence.
Collaborate with service users, carers and professionals to coordinate effective care and support.
Maintain accurate and up-to-date records in line with council and professional standards.
Provide advice, guidance and training to support adults and their carers in daily living activities.
Contribute to service development initiatives aimed at improving adult occupational therapy services.
