As an Occupational Therapist with Croydon Health Services NHS Trust, working within community rehabilitation and neurology services, you will provide specialist occupational therapy assessment and intervention to support patients’ recovery and independence. Collaborating closely with patients, families, carers and multidisciplinary teams, you will develop and deliver personalised rehabilitation programmes tailored to neurological needs.
Conduct comprehensive assessments of patients’ functional abilities and rehabilitation requirements.
Develop, implement and review tailored occupational therapy intervention plans in line with best practice.
Work collaboratively with patients, carers and professionals to coordinate care and facilitate access to community resources.
Maintain accurate and timely clinical documentation in accordance with NHS and professional standards.
Provide education, advice and training to support patients and carers in managing neurological conditions.
Contribute to service development and quality improvement initiatives within the occupational therapy service.
