Manchester City Council is recruiting a Band 6 Occupational Therapist to join the Reablement Service, supporting adults to regain independence and achieve their personal goals within the community.
Complete functional assessments to identify needs and set reablement goals.
Provide short-term interventions to promote independence and reduce long-term care needs.
Recommend and arrange equipment or adaptations to support safe daily living.
Work closely with multi-disciplinary teams, carers, and families to create tailored reablement plans.
Support hospital discharge and prevent avoidable admissions.
Maintain accurate records and contribute to the ongoing development of the reablement service.
