Job details:  Reablement. Experienced Occupational Therapist required to join Reablement team, must be prepared to travel within the South Wiltshire area. The Reablement Team have a positive impact on individuals’ lives following illness, injury, loss of confidence or hospital admission. Reablement offers Occupational Therapists the unique opportunity to work proactively with customers in their own homes to achieve greater independence, promote their well-being and minimise long-term care needs. The post holder will conduct strengths-based assessments, work with customers and their families to design and implement creative reablement plans to enable them to achieve their goals. The Reablement Service is led by occupational therapists alongside occupational therapy assistants and support workers, and benefits from support from the Principal Occupational Therapist. The post holder will be conducting visits to clients in their own homes or other settings, such as hospitals. JD available on request:
Undertake assessments, determine care packages/support plans and manage a large and complex caseload to maintain or improve the wellbeing of clients. Professionally supervise and support other colleagues. Skills/experience: minimum of two years post-qualification experience. Valid full UK driver’s license is essential. Hybrid working – at least 2 days working at the hub. Working hours: 37 hours per week
