Deliver occupational therapy services tailored to individual patient needs in the community.
Carry out best interest assessments as required.
Support patients with housing needs and moving and handling requirements.
Assess, recommend, and facilitate the use of equipment and adaptations.
Collaborate effectively with patients, families, and the wider team.
Maintain accurate records in line with professional and organisational standards.
