As an Occupational Therapist with Mid Cheshire Hospitals NHS Foundation Trust, working in community settings, you will provide occupational therapy services exclusively to adults. Operating at Band 6, you will support patients in achieving greater independence and improved daily living outcomes, working closely with families and multidisciplinary teams to deliver person-centred care.
Carry out thorough occupational therapy assessments for adults in community settings.
Design, implement and evaluate individualised therapy programmes tailored to each patient’s needs.
Collaborate with patients, families, carers and healthcare professionals to coordinate care and support.
Deliver occupational therapy interventions in line with professional standards.
Maintain accurate and detailed clinical records in accordance with organisational and professional guidelines.
Contribute to the ongoing development and improvement of community occupational therapy services.
