Job Title:  Community Occupational Therapist – Band 6
Area:  West Yorkshire NHS

Job Details: To start asap to help manage waiting lists.  
Skills/experience: essential – Minimum 2 years post registration.
Clinical experience working with Adults – ideally looking for a wide range of experience e.g. covering older people, frailty, falls, dementia, MSK, Neuro (PD, MS), medical, ortho.
To be able to independently hold, prioritise and manage caseload, long arm advice and supervision will be offered
Awareness/experience of MCA and unwise decisions.
Basic knowledge of equipment provision, aids and adaptations.
Car Driver, access to own car with business insurance, willing to commute to and across hub area.
Offering a minimum of 3 days per week, 4/5 days
Evidence of relevant and recent CPD.
Desirable – 3 years’ experience.
Community experience.
Waiting lists for long term conditions.
Hospital discharges, Rapids, prevention of admission.
Experience working with MDT and delegation to non-registered workforce.
Knowledge of holistic assessment including pressure management, nutrition and hydration
Awareness/experience of falls prevention & management.
Able to take clinical observations (BP, O2 sats, HR, Resp rate, lying/standing BP), use of NEWS 2.
Knowledge of deteriorating patient signs and symptoms.
Knowledge of palliative care/RESPECT.
Awareness of role of Neighbourhood teams and the nature of the work.
Previous experience of delivering rehabilitation of meal prep, personal cares.
Flexibility across teams.
Use of System 1/electronic records
Student supervision/practice educator training.
Working hours: Monday to Friday, 08.00-16.00pm, 8-5 and 8-6pm also considered
