Provide specialist occupational therapy assessments and interventions for adults within the community.
Support patients with complex health and social care needs, promoting independence and safe discharge.
Carry out moving and handling assessments, and recommend/provide equipment and adaptations.
Work collaboratively with the Integrated Community Enablement (ICE INT) Team and wider MDT.
Maintain accurate and timely clinical documentation in line with professional standards.
Band 6 locum role, ongoing contract, negotiable salary, working hours 08.00–16.00, Monday to Friday.
