Provide occupational therapy assessments and interventions for adults with acute or chronic long-term conditions at South Tyneside and Sunderland NHS Foundation Trust.
Develop and implement individualised treatment plans to support functional independence, rehabilitation, and quality of life.
Collaborate with multidisciplinary teams to ensure coordinated, person-centred care.
Maintain accurate clinical records and documentation in line with HCPC and organisational standards.
Apply clinical reasoning and evidence-based practice to guide therapy interventions and monitor patient progress.
Locum Band 6 role, starting ASAP for 3 months, 37.5 hours per week, Monday to Friday; HCPC registration and OT degree required.
