Deliver Telecare services to clients within the London Borough of Tower Hamlets, providing advice, support, and assistance to service users, customers, and partner agencies.
Respond efficiently as the first point of contact for calls requiring action, including coordinating with out-of-hours staff and other agencies.
Assess service user needs via the Jontek system, telephone, or in person, taking appropriate actions such as home visits, welfare checks, or providing first aid and personal care as required.
Manage Telecare equipment, including assessment, installation, programming, user instruction, troubleshooting, replacement, and removal.
Collaborate with colleagues and external agencies to implement and maintain high standards of Telecare and out-of-hours services.
Work flexibly within a 24-hour, 365-day rota, including shift-based and standard hours, to meet service requirements; locum role with ongoing contract.
