Job Title:  Specialist Occupational Therapist– Band 6

Job details:  Intermediate Care. Dual role: Specialist Occupational therapist required for Front of House and Intermediate Care Team. To provide rapid holistic therapy assessment of patients in the Emergency Department and Acute Admissions Unit, working with the MDT to prepare a patient for discharge and if needed community support. To start asap. JD available on request:

  • To work as an autonomous practitioner within the intermediate care service.
  • To be responsible for own caseload and undertake assessment of patients with acute, complex and multiple pathologies using specialist clinical reasoning skills.
  • To serve the acute NHS Trust in Emergency Department, Acute Assessment Unit and any other areas of the acute hospital to prevent admission to hospital and to expedite safe discharge.
  • To identify patient needs, agree goals and provide appropriate therapy interventions.
  • Provide adaptive equipment, rehabilitation programmes.
  • To keep the patient at the centre of care, ensuring patient and carer participation in decision making.
  • To provide support and education to peers, new staff non-registered staff and students.
  • To supervise junior staff and students, overseeing patient intervention and ensure Assistant Practitioners and Integrated Care Workers deliver high quality patient care.
  • To be actively involved in teaching and supporting Assistant Practitioners and Integrated Care Workers to achieve competencies.
  • To be accountable for a delegated caseload held by Integrated Care Workers and Assistant Practitioners.
  • To prioritise all referrals according to clinical need and to allocate or signpost appropriately.
  • To provide a high standard of care.
  • To work in various community settings, this includes lone working, with access to Advanced Practitioners when required.

Working hours: Monday to Friday. Front of House covers 08.00-16.00 and 10.00-18.00 and Intermediate Care Team cover 08.30 – 16.30, although hours can be flexible
