Job details: Physical Disability and Older People (PDOP). The PDOP Team works closely with other teams who are involved with the delivery of Adult Social Care.  The PDOP team forms part of the Living Well Service of ASC where the Mental Health and Learning Disability and Autism (LDA) team are also members. JD available on request.
The post holder will manage a caseload of individuals requiring functional assessments and identify necessary interventions which may include, manual handling, equipment and adaptations, to make a real difference to our service users. They will also be required to complete mental capacity assessment and risk assessments to ensure needs, wishes and best interests are identified.  Some support on the busy OT Duty may be required. The post holder will need to be available for home visits and be able to be at the base at least 2 days per week. Working hours: Full time.
