Social Care Assessor
Brief Job Overview
Tripod Partners are recruiting a motivated and enthusiastic Social Care Assessor for a locum opportunity with Enfield Council. Working within the North London Social Services, you will join the Integrated Learning Disabilities Service (ILDS), supporting young people with learning disabilities as they transition from Children’s Services into Adult Social Care.
Working alongside the Senior Practitioner leading the Transition Project, you will undertake assessments, develop person-centred support plans, review care packages, and work collaboratively with young people, families, schools, health professionals, and partner agencies to support successful transitions into adulthood.
Key Responsibilities
- Undertake social care assessments for young people with learning disabilities transitioning from Children’s Services into Adult Social Care.
- Develop person-centred and strengths-based support plans that promote independence and positive wellbeing.
- Review existing care packages and assess whether support continues to meet individual needs and outcomes.
- Work closely with young people and their families to understand their strengths, aspirations, needs, and desired outcomes.
- Liaise with schools, health professionals, and partner agencies to coordinate effective transition planning.
- Participate in safeguarding work and respond appropriately to identified risks or concerns.
- Participate in duty responsibilities as required by the service.
- Promote autonomy and support young people to develop the skills and opportunities needed for a fulfilling adult life.
- Work within a strengths-based, person-centred framework throughout assessment, planning, review, and intervention.
- Work as part of the fully integrated health and social care service alongside Social Workers, Community Nurses, Psychologists, Occupational Therapists, Speech and Language Therapists, Physiotherapists, Psychiatrists, and other specialist professionals.
- Maintain accurate and timely records in accordance with service requirements.
About You
You will be a passionate and committed Social Care Assessor with experience supporting people with learning disabilities. You will have a strong understanding of the importance of person-centred and strengths-based practice and be committed to helping young people achieve greater autonomy, wellbeing, and independence.
You will be confident working collaboratively with young people, families, education, health, and social care professionals. Experience of transition work or supporting young people moving from Children’s Services into Adult Social Care would be highly relevant to this role.
Requirements
- Relevant experience in social care assessment and support planning.
- Experience working with people with learning disabilities.
- Understanding of transition from Children’s Services into Adult Social Care.
- Experience undertaking assessments and reviewing care and support packages.
- Strong person-centred and strengths-based approach.
- Ability to develop effective support plans focused on individual outcomes.
- Experience working with families, schools, health professionals, and partner agencies.
- Understanding of safeguarding responsibilities.
- Ability to participate effectively in duty arrangements.
- Strong communication, assessment, organisational, and partnership-working skills.
- Commitment to supporting people with learning disabilities to live autonomous lives and reach their full potential.
