Job details:  Reablement. Motivated, dynamic individual required who is able to work well under pressure to achieve the best outcomes for Manchester citizens with a focus on planning, goal setting and ensuring outcomes are delivered in a timely manner. Must be a qualified Occupational therapist. HCPC registered. The candidate must have experience in equipment and adaptations and goal setting. There will be an opportunity to work closely with and deliver Goal setting training to Reablement support workers across the city. The role is city wide although the post holder will be predominantly based in the north of the city, therefore a car is essential. JD available on request:

• The role holder will work to provide direct support for service users and their families in accordance with statutory responsibilities and local and national policies and procedures in order to secure positive outcomes for the customer.
• The role holder will work with City Council colleagues, partner agencies and stakeholders to effectively manage and plan for present and future risk situations of service users.
• The role holder will assess and plan person-centred packages to meet the individual needs of service users to promote their independence.
• The role holder will supervise and develop students and trainees and actively contribute to the development of other staff.

Working hours: 35 hours per week. Monday to Friday, 09.00 – 16.30.
