Reablement unit, people who have been discharged from Hospital, discharge planning, dealing with families, home visits, equipment ordering, assessments, short term care packages. Car Driver required. Looking for band 6 or higher candidates. Office based 3-4 days – write-up can be WFH. Working hours: hours are flexible – could start aware between 7am and finish late – dependent candidate needs.
