Job Title: Occupational Therapist – Band 7 Location: Frimley Health – Fleet Community Hospital Area: Surrey NHS Salary: £negotiable Job Type: Locum Contract length: Ongoing Job details: Provide assessment, rehab and treatment to patients discharged on D2A pathway to care homes. Work with MDT (OT, GP, Care Home Matron, Social Care, Care Home staff) to implement rehab a dynamic assessment. Contribute to decision making about discharge destination. Assess, prescribe and advise on correct management for contractures and pressure relieving equipment. Provide guidance to care home staff on specialist seating. Audit and service development; relationship building with care homes and staff. Advice and teaching as appropriate Whilst the caseload will be predominantly working with patients within care homes, the OT will be required to have generalist community therapy skills to undertake routine community therapy and rehab with patients in their own homes (not in care homes).
