Undertake and oversee complex assessments, safeguarding enquiries, and care planning under the Care Act 2014 and Mental Capacity Act 2005.

Provide professional leadership, mentoring, and support to colleagues, contributing to a culture of reflective and strengths-based practice.

Collaborate with health, housing, and voluntary sector partners to deliver integrated, person-centred services.
