View JobLeeds is seeking a compassionate and proactive Hospital Discharge Social Worker to join the integrated discharge team. This role focuses on supporting adults to leave hospital safely and timely, ensuring continuity of care and access to community support. You will work closely with healthcare professionals, patients, and families to coordinate assessments, care packages, and promote independent living.
Key Responsibilities:

Undertake social care assessments of patients in hospital settings to determine discharge needs under the Care Act 2014.

Develop and coordinate person-centred discharge plans, collaborating with multidisciplinary teams, families, and community services.

Facilitate timely discharge by arranging appropriate care and support, including reablement, domiciliary care, and community resources.

Maintain accurate records and documentation in line with NHS and local authority standards and data protection policies.
