West Lothian Council is seeking a dedicated Locum Adult Social Worker to join their Adults Services team, focusing on care planning and support coordination. This role involves assessing adults with care and support needs, developing personalised care plans, and ensuring effective service delivery to promote independence and wellbeing.

Key Responsibilities:

  • Conduct comprehensive assessments of adults’ care and support needs.

  • Develop, implement, and review personalised care plans in partnership with service users and their families.

  • Coordinate services with health, housing, and community partners to meet identified needs.

  • Undertake safeguarding duties and ensure compliance with statutory guidance and local policies.

  • Advocate for service users and provide advice and support to promote choice and control.

  • Maintain clear, accurate, and timely records in line with West Lothian Council’s standards and statutory requirements.
