Join Newcastle City Council to undertake strengths-based, person-centred assessments for adults in line with the Care Act 2014 and other statutory frameworks.

Develop and review care and support plans that promote independence, wellbeing, and safeguarding, ensuring they are tailored to individual needs and aspirations.

Manage a varied caseload, including complex cases, while ensuring that statutory responsibilities are met and outcomes are achieved in a timely manner.

Identify and respond to safeguarding concerns, coordinate Section 42 enquiries, and contribute to the development of adult safeguarding plans.

Work in partnership with health, housing, mental health, and voluntary sector professionals to provide integrated, multi-agency support for adults with care and support needs.

Maintain high-quality records and documentation in accordance with Social Work England standards and Newcastle City Council’s policies and procedures.
