View JobTripod Partners are looking for experienced Advanced Community Practitioner to join a high-performing team delivering expert clinical care and case management in the community. You will work across multidisciplinary teams to reduce avoidable hospital admissions, facilitate safe discharges, and support personalised, proactive care for people with frailty.
Key Responsibilities:

Identify patients at high risk of admission and deliver early, proactive intervention.

Carry out holistic assessments and develop personalised or advanced care plans.

Liaise with GPs, social care, therapy colleagues, and mental health services.

Work as a Non-Medical Prescriber (NMP) to initiate and manage treatment plans.
-
Support patients with long-term conditions and complex health needs.
-
Contribute to Virtual Ward and discharge pathways.

Maintain clinical records, contribute to audits, and uphold service quality standards.

Provide clinical supervision, mentoring, and peer support.
Requirements:
-
Registered with NMC or HCPC.

NMP qualification
-
Strong experience in community frailty or acute care.
-
Proven ability to work autonomously and as part of an integrated team.
-
UK driving licence and access to a vehicle during the working day.
Rate: £30 per hour
Contract: Ongoing
Hours: Full-time
IR35 Status: Inside IR35
