Key Responsibilities:

Conduct Care Act assessments and reviews to develop person-centred care and support plans.

Work with adults with physical disabilities, learning disabilities, mental health needs, and older people.

Liaise with families, health professionals, and care providers to coordinate safe and effective support.

Carry out safeguarding enquiries and ensure adherence to relevant legislation and best practices.
-
Maintain clear, timely case recordings and contribute to multi-disciplinary planning and decision-making.
