Bristol City Council is seeking a compassionate and skilled Locum Qualified Social Worker to join their Leaving Care team for a 3-month contract. This role focuses on supporting young people transitioning from care to independent living, ensuring they receive the guidance and services needed to thrive.
Key Responsibilities:
- 
Manage a caseload of young people leaving care, supporting their transition to independence.
 - 
Develop and review Pathway Plans in line with statutory requirements and individual needs.
 - 
Liaise with housing, education, health, and employment services to provide coordinated support.
 - 
Advocate for young people to ensure their voices are heard and rights upheld.
 - 
Maintain accurate case records and ensure compliance with relevant legislation and local policies.
