Job details: Community Services. D2A. Skills/experience: The person requires community experience within adult reablement. The person must be a car driver and owner. Although the service works in geographical clusters, the individual should be willing to travel cross county if required to meet patient need. The D2A service provides short term reablement following hospital admission within peoples own homes. The post holder will work alongside D2A trained provider agencies to deliver assessment and reablement. They will set patient led SMART goals to progress independence and recommend longer term care and support needs. Working hours: shifts, 09.00 – 17.00.
