Occupational Therapist

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity with North Somerset Council. Working within South West Social Services, you will join the Adult Social Care Occupational Therapy Team, working closely with the Reablement Service to support Discharge to Assess pathways and help adults regain independence following hospital discharge.
This community-based role offers the opportunity to work with adults who have complex physical, functional, and social care needs, delivering person-centred Occupational Therapy assessments, moving and handling interventions, equipment provision, and care planning. You will play a key role in promoting safe hospital discharge, preventing unnecessary admissions, and enabling people to live safely and independently within their own homes.
Key Responsibilities
- Undertake comprehensive Occupational Therapy assessments for adults within the community.
- Support the Discharge to Assess pathway by working alongside the Reablement Service to facilitate safe and timely hospital discharge.
- Complete moving and handling risk assessments and develop safe, practical solutions for service users and carers.
- Carry out Mental Capacity Assessments and Best Interests decisions in accordance with the Mental Capacity Act.
- Manage a varied Adult Social Care caseload, undertaking care assessments and ongoing case management.
- Develop person-centred Occupational Therapy goals that promote independence, recovery, and wellbeing.
- Assess for and recommend specialist equipment, assistive technology, and home adaptations to maximise independence.
- Work collaboratively with Reablement staff, Social Workers, healthcare professionals, and other multidisciplinary partners.
- Maintain accurate electronic case records and ensure all documentation complies with legislative and professional standards.
- Promote strengths-based practice and support individuals to achieve the best possible outcomes within their own homes.
About You
You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care, community Occupational Therapy, or discharge services. You will have strong assessment and clinical reasoning skills, together with experience in moving and handling, equipment provision, and strengths-based practice.
You will be confident working independently across the community while collaborating effectively with multidisciplinary teams to deliver high-quality, person-centred care.
Requirements
- Degree or Diploma in Occupational Therapy.
- Current HCPC registration.
- Experience working within Adult Social Care or community Occupational Therapy services.
- Experience supporting Discharge to Assess or reablement pathways is desirable.
- Knowledge of the Mental Capacity Act and experience completing Mental Capacity Assessments and Best Interests decisions.
- Experience undertaking moving and handling risk assessments.
- Knowledge of specialist equipment, assistive technology, and home adaptations.
- Strong case management, assessment, communication, and multidisciplinary team-working skills.
- Ability to travel throughout the North Somerset area for community visits.
