Reablement Assistant

Job Overview

 

Reablement Assistants required to support service users in regaining and maintaining independence. Candidates must have an up-to-date face-to-face Moving & Handling certificate, a minimum of 5 years working history, and must not require sponsorship.

 

Main Areas of Responsibility

 

  •  
  • Meet the physical, social, and emotional needs of service users as agreed in care plans to promote independence and enhance living skills
     

  • Provide personal care support to service users
     

  • Assist service users with tasks as agreed in support plans
     

  • Work with individuals to improve and maintain daily living skills, using appropriate equipment when required
     

  • Liaise and work alongside other professionals to minimise risk and enable individuals to remain in their own homes
     

  • Work in partnership or as part of an integrated team of allied health and social care professionals
     

  • Work alongside service users in the implementation of individual, person-centred support plans and participate in assessments and reviews as required
     

  • Promote good care practice, including empowerment, dignity, independence, and respect for cultural, spiritual, and religious needs
     

  • Maintain appropriate records and report any significant changes in service users’ circumstances to the Line Manager
     

  • Work as part of a team, Monday to Friday, 09:00 – 16:00
     

  • Support and participate with service users in activities and recreational pursuits
     

  • Be flexible and contribute to the changing needs of the service as required
     

  • Carry out all duties in accordance with City Council policies, including anti-discriminatory practice and equal opportunities
     

  • Ensure compliance with statutory requirements, Standing Orders, Financial Regulations, health and safety legislation, codes of practice, and the City Council’s safety plan
