Carry out statutory duties for North Lincolnshire Council under the Care Act 2014, undertaking strengths-based assessments and reviews for adults with care and support needs.

Manage a caseload involving older adults, people with physical disabilities, learning disabilities, or mental health needs, promoting independence and personal outcomes.

Develop personalised care and support plans that meet eligibility criteria while empowering individuals to live safely and independently in their communities.

Undertake safeguarding enquiries under Section 42 of the Care Act, coordinating protection plans and working closely with partner agencies.

Apply the principles of the Mental Capacity Act 2005, completing capacity assessments and participating in best interest decisions as required.

Maintain accurate, timely case records and contribute to service development by engaging in supervision, training, and reflective practice.
