Contract: Locum, Ongoing

About the Role

An experienced Occupational Therapist is required to support an Adult Care Review Team on a fast-paced project-based assignment. You will work alongside an Advanced Practitioner and Social Workers, reviewing existing packages of care within Supported Living settings and ensuring a strength-based, incremental approach to care.

Main Responsibilities

  • Assess service users with existing care packages in supported living properties.
  • Apply a strengths-based, proportionate approach to reviewing care and support.
  • Complete occupational therapy assessments and provide advice to promote independence.
  • Recommend and provide equipment and minor adaptations as appropriate.
  • Undertake moving and handling assessments where required.
  • Evaluate the effectiveness and appropriateness of current care and support packages.
  • Work collaboratively with community care providers to ensure needs are met.
  • Establish effective working relationships with internal teams and external agencies.
  • Ensure resources are commissioned and used in a cost-effective and efficient manner.
  • Maintain accurate, timely records in line with legislation and council policies.

Skills & Experience Required

  • Degree in Occupational Therapy and HCPC registration.
  • Significant experience working within Adult Social Care.
  • Strong understanding of the Care Act 2014.
  • Experience reviewing and evaluating care and support packages.
  • Competence in equipment provision, minor adaptations and moving & handling.
  • Ability to work effectively within a local authority setting and multidisciplinary team.
  • Excellent communication, organisation and decision-making skills.

Working Hours

  • Part-time hours considered.

Support will be provided by the Project Lead and Community Occupational Therapy Managers, making this a great opportunity to contribute to a focused adult care review project.
