Complete holistic occupational therapy assessments for adults with complex health and social care needs.

Deliver interventions in respiratory care, general medicine, and rehabilitation, supporting recovery and long-term independence.

Provide recommendations and arrangements for equipment and adaptations to promote safe discharge and daily living.

Work collaboratively within a multidisciplinary team to deliver coordinated, patient-centred care across acute and community services.

Contribute to discharge planning, ensuring continuity of care between hospital and community settings.

Support service improvement and share specialist knowledge within occupational therapy practice.
