About the Role
Medway NHS Foundation Trust is seeking a Band 6 Physiotherapist to join the Older Persons / Elderly Frailty service. This role focuses on delivering high-quality physiotherapy assessment and intervention to older adults, supporting mobility, rehabilitation and safe discharge planning within an acute hospital setting.
Main Responsibilities
- Provide physiotherapy assessment and treatment for older adults with frailty and complex needs.
- Develop and implement rehabilitation and mobility programmes tailored to individual patient needs.
- Support falls prevention, functional recovery and safe discharge planning.
- Work collaboratively within the multidisciplinary team to optimise patient outcomes.
- Maintain accurate and timely clinical documentation in line with Trust and HCPC standards.
Skills & Experience Required
- HCPC-registered Physiotherapist.
- Experience working with older people and/or frailty services.
- Strong assessment, clinical reasoning and rehabilitation skills.
- Ability to work effectively within a multidisciplinary team.
Working Hours
- Shift pattern: 08:00 – 16:00.
