Job Details:
- Occupational Therapist required to provide an outcome-focused assessment service for adults with physical disabilities and age-related conditions.
- Deliver a professional Occupational Therapy and Reablement service, including statutory social care responsibilities.
- Carry out person-centred, strengths-based assessments to promote independence and recovery.
- Work collaboratively with multidisciplinary teams and partner agencies to support individuals with complex needs.
- Support safe and timely hospital discharges in partnership with health colleagues.
- Maintain accurate and up-to-date case records in line with service standards.
Skills / Experience:
- Qualified Occupational Therapist with HCPC registration.
- Experience working with adults with physical disabilities and age-related conditions.
- Experience within a social care or community setting.
- Knowledge of reablement and outcome-focused practice.
- Strong communication and multidisciplinary working skills.
- Ability to manage a caseload independently.
