Occupational Therapist

Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity within South East London Social Services. Based with the London Borough of Bexley, this role sits within the Integrated Rehabilitation Service, supporting adults to regain independence through reablement, provision of minor adaptations, and specialist equipment.

This is an excellent opportunity for an Occupational Therapist with experience in Adult Social Care or community rehabilitation who is passionate about enabling people to remain safe and independent in their own homes. You will work as part of a multidisciplinary team, delivering person-centred interventions that maximise functional ability, reduce reliance on long-term care, and improve quality of life.

Key Responsibilities

  • Undertake comprehensive Occupational Therapy assessments within the community using a person-centred, strengths-based approach.
  • Develop and implement reablement plans that promote independence and functional recovery.
  • Assess for and prescribe minor adaptations, assistive equipment, and daily living aids to support safe independent living.
  • Review the effectiveness of interventions and adapt care plans to meet changing needs.
  • Provide advice and education to service users, families, and carers to maximise independence and wellbeing.
  • Work collaboratively with health and social care professionals within the Integrated Rehabilitation Service.
  • Support timely hospital discharge and admission avoidance where appropriate.
  • Maintain accurate, timely, and professional case records in accordance with HCPC and local authority standards.
  • Contribute to positive multidisciplinary working and the ongoing development of the service.

About You

You will be an HCPC-registered Occupational Therapist with experience in Adult Social Care, reablement, or community rehabilitation. You will be confident assessing functional needs, recommending equipment and minor adaptations, and delivering interventions that promote independence and reduce risk.

You will possess excellent assessment, communication, and clinical reasoning skills, together with a proactive and collaborative approach to multidisciplinary working.

Requirements

  • Degree or Diploma in Occupational Therapy.
  • Current HCPC registration.
  • Experience within Adult Social Care, community rehabilitation, or reablement services.
  • Experience assessing for and prescribing minor adaptations and specialist equipment.
  • Strong understanding of person-centred and strengths-based practice.
  • Excellent communication, organisational, and multidisciplinary team-working skills.
  • Ability to manage a community-based caseload independently.
  • Knowledge of relevant Adult Social Care legislation and best practice is desirable.
