Job Title: Reablement Assistant (Multiple Roles)

Job Details:
- Reablement Assistants required to undertake visits across the city
- Provide hands-on care and support using a strengths-based approach to achieve reablement goals
- Deliver short-term social care reablement promoting independence, wellbeing, choice and dignity
- Follow outcomes-focused reablement programmes prescribed by a Reablement Assessor or registered professional
- Support hospital discharge, hospital avoidance, crisis intervention and carer breakdown in the community
- Engage in strengths-based conversations with individuals and carers to promote independence, health and wellbeing
- Manage risk and prevent harm in line with Making Safeguarding Personal principles, keeping the person at the centre of planning
- Contribute to effective delivery of adult social care services in line with statutory responsibilities and best value principles
Working Pattern:
- 7-day service operating between 06:30 – 22:00
- Shift pattern to include:
- 2 x 07:00 – 13:00
- 1 x 08:00 – 14:00
- 2 x 17:00 – 22:00
- 1 x 16:00 – 21:00
- 2 x 07:00 – 13:00
- Required Monday to Sunday
- Regular staffing required across an 8-week period
