The successful candidate will work collaboratively with various stakeholders to ensure the safety, well-being, and development of children in the LAC system.
Key Responsibilities:
- Conduct Assessments: Perform comprehensive assessments of children in care to identify their needs and develop effective care plans
- Case Management: Manage a caseload of LAC cases, ensuring timely interventions and ongoing support
- Multi-Agency Collaboration: Work closely with health professionals, educators, foster carers, and other agencies to coordinate efforts and share relevant information
- Advocacy: Advocate for the rights and best interests of children in care, ensuring their voices are heard and respected in all processes
- Documentation: Maintain accurate and detailed records of all activities, ensuring compliance with legal and regulatory requirements
