This role involves working with adults experiencing a range of mental health issues, providing holistic assessments, safeguarding, and care planning to support recovery, independence, and wellbeing. You will work collaboratively with health professionals, service users, and families to deliver person-centred mental health care within the community.
Key Responsibilities:

Conduct comprehensive mental health assessments in line with the Care Act 2014 and the Mental Health Act 1983.

Manage safeguarding enquiries and interventions for adults at risk of abuse or neglect.

Develop and implement person-centred care and support plans in partnership with service users and multi-agency teams.

Apply the Mental Capacity Act 2005 to assess decision-making abilities and support best interest decisions.

Maintain accurate and up-to-date case records and reports in compliance with Harrow Council policies and statutory requirements.
