Hammersmith and Fulham Council is seeking a compassionate and skilled Social Worker to join our Looked After Children (LAC) Team. This role involves providing care, support, and advocacy for children and young people in the care system. The successful candidate will work collaboratively with carers, families, and partner agencies to promote permanence, stability, and positive outcomes for looked after children.
Key Responsibilities:

Manage a caseload of looked after children, conducting regular statutory visits and ensuring care plans are up to date and child-centred.

Prepare and contribute to care plans, permanency planning, and Looked After Reviews in line with statutory guidance.

Support children through transitions including placement changes, education, and preparation for independence.

Attend and contribute to multi-agency meetings, including Child Protection Conferences and court hearings.

Maintain accurate, timely case records and reports in compliance with Hammersmith and Fulham Council policies.
