Key Responsibilities:

Lead and manage a team of social workers and practitioners focused on hospital discharge and post-discharge support.

Oversee complex casework, ensuring Care Act compliance and effective multi-agency coordination.

Drive performance improvement, ensuring timely assessments and discharges within legal and NHS framework requirements.

Provide supervision, mentorship, and support to staff, fostering professional development and resilience.

Maintain quality assurance, service delivery standards, and adherence to safeguarding procedures.
