Carry out statutory duties under the Care Act 2014, including strengths-based assessments, care planning, reviews, and safeguarding adults at risk of harm.

Support older people, adults with physical or learning disabilities, and those with mental health conditions, enabling them to live safely and independently.

Work closely with health professionals, housing services, and voluntary sector partners to provide integrated, multi-agency support.

Maintain a caseload of complex work, while ensuring that interventions are timely, legally compliant, and outcome-focused.
