Manage complex and high-risk adult social care cases for Brent Council, including safeguarding, hospital discharge, mental health, and continuing healthcare pathways.

Support and mentor social workers and newly qualified staff, offering practice guidance and contributing to professional development across the team.

Undertake strengths-based assessments under the Care Act 2014, creating outcome-focused support plans that promote independence and personal wellbeing.

Lead or contribute to Section 42 safeguarding enquiries, working with multi-agency partners to ensure timely and proportionate risk management.
