Carry out statutory duties under the Care Act 2014, including assessments, support planning, and reviews for older people, individuals with disabilities, or mental health needs.

Manage a varied caseload, applying strengths-based and outcome-focused approaches to promote independence and well-being.

Undertake safeguarding enquiries (Section 42) and ensure timely, legally compliant interventions that protect vulnerable adults from harm.

Work collaboratively with multi-disciplinary partners including health, housing, voluntary sector, and carers to deliver effective, integrated care solutions.
