- Occupational Therapist required to work within a multidisciplinary team including Social Workers, Occupational Therapists and Physiotherapists
- Provide integrated health and social care services to the local community
- Care coordinate a caseload of individuals with varying levels of complexity
- Lead on coordinating care and support for service users and their carers/families, involving other professionals where appropriate
- Support colleagues with complex cases where required and directed by the Team Manager
- Maintain accurate and comprehensive case management records for monitoring and planning purposes
- Provide an efficient and cost-effective service using appropriate resources from Social Services, Health, Council services and community organisations
- Adhere to Health and Care Professions Council (HCPC) standards of proficiency
- Maintain Continuous Professional Development (CPD) and attend post qualification training as required
- Work flexibly where required to meet the needs of the service, potentially including a 7-day service model
