Deliver statutory adult social care services for Brent Council, supporting adults with care and support needs under the Care Act 2014.

Complete strengths-based assessments, support planning, and reviews for individuals with physical disabilities, learning disabilities, mental health conditions, or age-related needs.

Manage a varied caseload, balancing preventative work, crisis intervention, and complex case management.

Undertake Section 42 safeguarding enquiries, working collaboratively with health, housing, and other partners to ensure effective protection planning.

Apply the Mental Capacity Act 2005 in everyday practice, including conducting capacity assessments and participating in best interest decisions.

Maintain accurate and timely case records, contribute to multi-disciplinary meetings, and engage in supervision and ongoing professional development.
