Service: Adult Social Care – Adaptations
Job Details:
- Occupational Therapist required to support Adult Social Care adaptations service.
- Carry out occupational therapy assessments for adults requiring home adaptations.
- Assess for and recommend major and minor adaptations to improve accessibility and safety within the home.
- Work collaboratively with housing services, surveyors and multidisciplinary teams to implement adaptation solutions.
- Support individuals to maximise independence and remain safely within their homes.
- Maintain accurate case records and reports in line with service requirements.
Skills / Experience:
- Qualified Occupational Therapist with HCPC registration.
- Experience working within Adult Social Care.
- Experience assessing for housing adaptations.
- Knowledge of equipment provision and environmental adaptations.
- Ability to manage a caseload independently.
- Strong communication and multidisciplinary working skills.
Working Pattern: Hybrid working available.
