Service: Acute Medical Unit
Job Details:
- Band 6 Occupational Therapist or Physiotherapist required to start ASAP within an Acute Medical Unit.
- Provide assessment, treatment and rehabilitation for patients with acute medical conditions.
- Support discharge planning to ensure safe and timely transitions from hospital to community settings.
- Work collaboratively within a multidisciplinary team to optimise patient outcomes.
- Manage a varied and fast-paced caseload within an acute environment.
- Maintain accurate clinical documentation in line with NHS standards.
Skills / Experience:
- HCPC registered Occupational Therapist or Physiotherapist.
- Experience working within an acute hospital or medical ward setting.
- Experience in discharge planning and rehabilitation.
- Ability to work effectively in a fast-paced environment.
- Strong communication and multidisciplinary team working skills.
Working Hours: Full time.
