Brief Job Overview

Tripod Partners are recruiting an experienced Occupational Therapist for a locum opportunity with Croydon Council. Working within South London Social Services, you will deliver high-quality Occupational Therapy services to adults within the community, helping people maximise their independence, improve safety, and remain living in their own homes.

This community-based role offers the opportunity to work autonomously while collaborating with Social Workers, healthcare professionals, housing teams, and other partner agencies. You will undertake holistic assessments, recommend equipment and adaptations, and develop person-centred interventions that promote independence and improve outcomes for adults with a wide range of physical and functional needs.

Key Responsibilities

• Undertake comprehensive Occupational Therapy assessments within community settings.
• Carry out home visits to assess functional ability, environmental risks, and support needs.
• Develop person-centred intervention plans that maximise independence and promote safe living.
• Assess for and recommend specialist equipment, assistive technology, and minor or major home adaptations.
• Provide practical advice and guidance to service users, carers, and families to support independent living.
• Work collaboratively with Social Workers, healthcare professionals, housing teams, and other partner agencies to coordinate care.
• Support hospital discharge and community rehabilitation where appropriate.
• Maintain accurate electronic case records and ensure compliance with relevant legislation, professional standards, and council policies.
• Manage an autonomous community caseload while prioritising work effectively.
• Promote strengths-based practice and positive outcomes for adults with care and support needs.

About You

You will be a HCPC-registered Occupational Therapist with experience working within Adult Social Care or community Occupational Therapy services. You will have excellent assessment and clinical reasoning skills together with a passion for supporting adults to live independently within their own homes.

You will be confident working independently within the community while maintaining strong working relationships with multidisciplinary colleagues and partner agencies.

Requirements

• Degree or Diploma in Occupational Therapy.
• Current HCPC registration.
• Experience working within Adult Social Care or community Occupational Therapy.
• Experience undertaking community-based functional assessments and home visits.
• Knowledge of equipment provision, assistive technology, and housing adaptations.
• Strong assessment, clinical reasoning, and case management skills.
• Excellent communication and multidisciplinary team-working skills.
• Ability to work autonomously and manage a varied community caseload.
