View JobTripod Partners are looking for a Band 7 Nurse to join a Continuing Healthcare (CHC) team in Mid and South Essex. This role is suitable for an experienced CHC Nurse, Advanced Nurse, Team Manager, or Specialist with strong knowledge of the CHC process. The successful candidate will support assessments, decision-making, and care planning for individuals eligible for NHS Continuing Healthcare funding.
Key Responsibilities:

Conduct CHC assessments and reviews in line with the National Framework.

Support decision-making processes for eligibility and care planning.

Liaise with multidisciplinary teams and external agencies.
-
Ensure timely documentation and compliance with clinical governance standards.

Support best practice in relation to CoPDoLS where applicable.
Position Details:
Location: Mid and South Essex
Duration: 3 months
Hours: Full-time (1WTE)
Pay Rate: £30 per hour
Work Pattern: Monday to Friday
Requirements:

NMC registered Nurse (essential).
-
Extensive CHC experience (essential).
-
Knowledge of Court of Protection and Deprivation of Liberty Safeguards (desirable).
-
Strong assessment, communication, and decision-making skills.
-
Right to work in the UK.
