This role focuses on supporting adults with neurological conditions through functional rehabilitation, equipment provision, and environmental adaptations. It offers the opportunity to make a real difference to residents’ independence and quality of life within a supportive and collaborative local authority setting.
Key Responsibilities:

Deliver person-centred occupational therapy assessments for adults in community settings, with a particular focus on neurological rehabilitation.

Develop and implement rehabilitation plans aimed at maximising functional independence and improving quality of life.

Recommend and arrange the provision of specialist equipment to support daily living, mobility, and safety at home.

Assess for and advise on minor and major home adaptations to promote safe and independent living.

Work collaboratively with service users, families, housing teams, and health professionals to deliver integrated support.

Maintain accurate clinical records and contribute to service planning, audits, and continuous improvement initiatives.
