Occupational Therapist – Discharge to Assess Team

Job Overview

We are seeking an experienced Occupational Therapist to join a busy Discharge to Assess (D2A) Team within North East London Social Services. This is an excellent locum opportunity for an Occupational Therapist who is passionate about supporting people to transition safely from hospital to home while promoting independence and preventing unnecessary admissions.

Working as part of a locality-based multidisciplinary team, you will provide integrated community health and adult social care services, carrying out holistic, person-centred occupational therapy assessments and recommending appropriate interventions to help individuals achieve the best possible outcomes.

Key Responsibilities

  • Undertake comprehensive occupational therapy assessments for adults with a range of health conditions.
  • Manage a caseload of approximately four cases per week, including both standard and complex occupational therapy assessments.
  • Assess, recommend and review simple and complex equipment to promote independence and safe living.
  • Complete manual handling assessments and provide appropriate recommendations.
  • Recommend major adaptations where required and contribute to housing-related solutions.
  • Develop person-centred support plans and carry out regular reviews.
  • Work collaboratively within a multidisciplinary team, including health and social care professionals.
  • Provide education and guidance on the safe use of equipment in accordance with legislation and best practice.
  • Maintain accurate records and manage complex cases effectively while meeting service deadlines.

About You

You will be a motivated and organised Occupational Therapist with excellent clinical reasoning skills and the ability to work independently while contributing effectively within a multidisciplinary environment. You will be confident managing complex cases and committed to delivering high-quality, person-centred care.

Requirements

  • Professional Occupational Therapy qualification recognised by the HCPC.
  • Current HCPC registration.
  • Experience working within adult social care and community occupational therapy services.
  • Excellent knowledge of specialist equipment, major adaptations and manual handling.
  • Experience assessing for and recommending both simple and complex equipment.
  • Strong understanding of adult social care legislation, the legal framework and housing legislation.
  • Experience undertaking holistic, person-centred assessments, support planning and reviews.
  • Ability to manage, prioritise and progress complex caseloads autonomously.
  • Experience working within a multidisciplinary team.
  • Excellent communication, assessment and report-writing skills.
